When you arrive, our friendly team will welcome you. We'll complete a few brief forms to better understand your needs and ensure we have all the right information to support you.
Meeting Your Provider
You’ll meet with a compassionate nurse practitioner in a safe, private space. This is a time for shared conversation about your history, your challenges, and your hopes for the future.
Collaborative Plan
Together, we’ll create a personalized care plan. Whether it involves medication management or wellness strategies, you are an active partner in every decision we make for your health.
Prepare for Success
A valid form of photo identification (Driver's License or Passport)
Your current insurance card and any required co-payment
A list of all current medications, including dosage and frequency
New patient forms completed through our secure online portal.
Please arrive 10–15 minutes early to your scheduled appointment.

Do you offer virtual appointments?
Yes, we provide secure telehealth visits so you can meet with your provider from the comfort and privacy of your home.
Are my sessions confidential?
Absolutely. We prioritize your privacy and strictly follow HIPAA guidelines to ensure all your personal and health information is protected.
How long do appointments last?
Initial evaluations typically take 60 minutes, while follow-up medication management visits are approximately 15-20 minutes.
What insurances do you accept?
We accept Aetna, United Health Care, Medicare, and Blue Cross Blue Shield. Private pay is available: $250 for initial visits and $125 for follow-up visits.
